I hereby certify that I am physically fit and can medically participate in the Community Gun Violence Awareness Walk. On behalf of myself, I hereby waive and discharge Jayz Agnew Foundation, its sponsors, and affiliates from any liability or claims that may occur as a result of my participation, including but not limited to loss, damage, injury, or death.

PHOTO DISCLAIMER

I hereby agree, consent, and authorize Jayz Agnew Foundation to use my likeness in any photograph, video, or other digital media taken on Saturday, November 16, 2024, during the Community Gun Violence Awareness Walk in any and all publications, including but not limited to, print or web-based publications.
